Patient privacy & accessing clinical records
How we collect and use your health information
Allevia Kensington Hospital complies with the Health Information Privacy Code 2020. The code recognises that people expect their health information:
- To be kept confidential, as it’s often shared in a situation of trust.
- To be treated with sensitivity, as it may include details about your body, lifestyle, emotions, or behaviour.
- Remain relevant over time, as it may be useful for your care even years after it’s collected.
- Be used only for the purposes it was collected for — and that you’re informed about those purposes.
To provide safe, effective care and continuity across your treatment, we may collect and share relevant information with other healthcare professionals involved in your care. This could include:
- Your specialist(s)
- Laboratories or diagnostic providers
- Other hospitals or healthcare services
- Your nominated General Practitioner (GP)
We’re also required by law to share certain health information with organisations like the Ministry of Health.
We may review your information before you arrive to ensure smooth coordination of your care. After your treatment, we may use this information for internal quality reviews and audits — all with the aim of improving the care we provide to you and others.
How to access or update your medical records
You have the right to request a copy of your medical records — or the records of someone you are legally authorised to represent — and to ask for corrections if anything is inaccurate.
- Download and complete our Request for Clinical Records form.
- Include a copy of signed photo ID (e.g. passport or driver's licence)
- Return your form and ID by hand, post, or email (details are on the form).
- Our staff may contact you to confirm your request and identity.
We are required to respond to your request within 20 working days once all necessary documents are received.
For help or more information on requesting your clinical records, please contact our reception via email at clinicalrecords@alleviakensington.co.nz or phone 09 437 9080.
Taking photos, videos or audio recordings
During your visit, we kindly ask that you do not take photos, videos, or audio recordings inside the hospital. This helps us protect the privacy of all patients, visitors, and staff.
